FOSTER INFORMATION AND APPLICATION

Are you ready to find out what is involved in fostering a service dog? You will find everything you need to know in the following documents. Start with “How Fostering Works” and then check out the “PAD Command List” and the “Allowed & Not Allowed” files.

Please download these documents and save them for your records – you will need to refer to them when working with your foster dog:

ALLOWED AND NOT ALLOWED SERVICE DOG ACTIVITIES

After you have a better understanding of how it all works it is time to complete the “Foster Application” and the “Goals Survey.” Send those two documents back to us and a representative from PAD will reach out to you regarding your application so that we can make sure that we select a dog that will fit into your home and lifestyle.

Please complete both of these documents and return them to PAD  – return in person, by mail, email, or fax. Please be aware that our representatives often work off-site and may not be able to meet with you without an appointment. 

APPLICATION TO FOSTER A SERVICE DOG & GOALS SURVEY

When you come to the facility to pick up your foster dog, please bring a copy of the “Foster Contract” and the “Foster Checkout List” so that they can be completed with a PAD representative.
