Thank you for volunteering to mentor one of PAD’s service dog teams. This form is used as a log to document your contacts with the team.  Please record each contact with the team so we can document the follow ups for Assistance Dogs International accreditation.  The entries may be brief if the team is not experiencing any challenges.  Provide more detail if there are any problems or concerns. Remember, it is the handler’s responsibility to contact you weekly.  Please coach/remind them to call in.  If they remain noncompliant, please contact the PAD office as this weekly contact is part of their contractual agreement.  

Your Veteran should be contacting you weekly until Public Access Testing is passed. These logs will document that our Veteran teams are staying in contact and are receiving ongoing support from Patriot Assistance Dogs.  Thank you for your help in making this program work!
